Drivers holding a Commercial Drivers License (CDL) are required by Federal Law to undergo a medical exam by a nationally certified medical examiner. This exam will include testing of visual acuity, hearing, urinalysis, vital signs, and a medical history and physical. If you use glasses, contact lenses, or hearing aids please bring these items with you to your exam.
In order to make your visit with us more efficient please complete the CDL medical exam form, print it, and bring it to your appointment.
Under normal circumstances your CDL medical exam will be valid for 2 years. However, depending on an individual’s personal history the medical clearance may be issued for a shorter time.
